New Patient Registration
If you live in the practice area and would like to become a patient of South East Family Practice. You need to complete a BSO Registration form you can download and print this by clicking on the link below or ask at reception. You should complete the appropriate form fully and leave at reception.
Types of Forms:
- HSCR1 form – for any new patient coming from outside of the UK, South Ireland OR if they haven’t been registered with a GP practice in over 1 year
- HS200 form – for any new patient previously registered within the UK (Inc. temporary patients)

Practice Area
The practice area covers a five-mile radius from the surgery, east of the River Lagan to Purdysburn Village, Carryduff, Moneyreagh, Gransha, Gilnahirk, Tullycarnet, Dundonald and Sydenham. There is a regular bus service within 100 yards of the surgery.
Practice Access
The practice premises have access and facilities for all disabled persons.
Non-NHS Patients
Private GP appointments for patients ineligible for NHS services (eg. Some overseas patients) are available to book online at: www.belfastprivategp.com
